Second Mortgage Program for Energy Conservation Checklist

The following forms are enclosed and need to be returned to AHFC to complete your application package.

1. Child Support Verification: Sign your name(s) in Part 1 on the lines(s) under “signature”.
Do not complete PART II of the form. The form is required by law on all borrowers.

2. Federal Equal Credit Opportunity Act: Read, sign your name(s) and date.

3. Statement Regarding Loan Application: Read, sign your name(s) and date.

4. Waiver and Release of Liability of the Corporation: Read, sign your name(s) and date.

5. Borrower’s Signature Authorization Form: Read, sign your name(s) and date.

6. Cost Breakdown: This form is to be completed UNLESS substantially the same information is otherwise provided, such as a contractor’s bid or supplier’s list.

7. Application: Fill out as completely as you can. If you do not understand any portion, please contact Deborah Sims, AHFC Mortgage Assistant II, at 800-478-2432, extension 8444 or by e-mail: dsims@ahfc.state.ak.us and she will assist you.

In addition to the above forms, please send the following information.

___ Copies of last 2 years tax returns, signed, complete with all schedules and W2’s.

___ Copies of last 2 pay check stubs. If self-employed, year-to-date Profit and Loss.

___ Copies of last 2 months bank statements, both checking and savings.

___ Proof you own your property, i.e. copy of the Warranty Deed or other similar documentation that shows your name in title and legal description.

___ Copy of Energy Rating/Audit and list of selected energy improvements. Include a sketch and/or photos if needed.

___ Complete list of material and prices needed to complete your project, including any contracted labor. This may be an estimate from a supplier or your own list. If your own list, tell us how you determined the cost. You may also use the Cost Breakdown form provided in this package.

___ If you are doing the labor yourself, provide information regarding past experience or other explanation documenting your ability to do the work required.

___ Photo(s) of your home, in addition to photos of your project.

___ Copy of your last property tax assessment, if you are in a taxing area.

___ Include a check made payable to Alaska Housing Finance Corporation for the Credit Report -$21.25 (one borrower) or $22.00 (two borrowers).
